It starts when Hulk manages to survive a sequence of cosmic adventures, like escaping a black hole and confronting the planet-eating Galactus. The idea of World-Breaker Hulk was popularized by the World War Hulk storyline. That incarnation of Hulk was already regarded as one of the strongest versions of the character, who could destroy cities with little effort.
