Viewers barely have time to process what’s happening before the reality sinks in; he’s gone. Now, everyone is asking the same question: what really killed him? While the show gives a brief answer, the truth behind John’s death goes deeper than one medical term. To understand what happened and why the series handled it this way, we have to look closer at the story that inspired it and listen to the actor who played him.
What Really Killed John Stirling In ‘Bridgerton’ Season 4?

In the final moments of episode 6, the show reveals that John has a headache and within the blink of an eye, he dies. The show does not pause to explain what happened medically. Instead, it moves straight into the emotional fallout. Because of that, the audience feels the same confusion Francesca does. One moment, she has a husband, and the next, she is standing at his funeral.

Then, in episode 7, the focus moves entirely to grief. Francesca tries to hold herself together during the funeral, but the cracks are visible. More importantly, she carries a heavy sense of guilt. She believes she failed John by not giving him a child. As a result, his death feels even more final; she has no living piece of him left behind. However, while the series avoids spelling out the cause, the books provide more context.
In Julia Quinn’s novels, John dies from what is understood to be a brain aneurysm. Just like in the show, it happens quickly. He complains of a headache, lies down, and never wakes up.
Why The Cause Of John’s Death Remains Unclear

At the same time, it’s important to remember the historical setting. Bridgerton takes place in Regency-era England, during the early 19th century. Back then, doctors did not have the knowledge or tools to properly diagnose conditions like brain aneurysms. So even if that was the cause, no one in that world would have been able to name it.

Julia Quinn has addressed this, explaining that “nothing could have been done to save him.” She has also pointed out that an autopsy would have been highly unlikely in that period. Because of that reality, John’s family would never have received a clear medical answer. As Quinn described it, all Francesca would ever know is that her husband had a headache, lay down, and died.
By choosing not to over-explain, the Netflix adaptation stays true to that logic. Interestingly, he knew early in production that his character would die. Still, he refused to let that knowledge influence his performance. As he explained in an interview with Cosmopolitan, “John doesn’t know he’s gonna die. I know he is, but he doesn’t know.” Because of that mindset, he played every scene with sincerity and no hint of what was coming.
Furthermore, Alli stayed on set while the cast filmed John’s funeral scenes. After they completed their emotional performances, he surprised them by stepping back in. He later shared that there were tears, followed by laughter, and then more tears again, calling it “the most amazing moment ever.”
