For a show that has survived two decades and built one of television’s most loyal fanbases, ‘Grey’s Anatomy‘ has surprisingly kept its spin-off universe small. While it did branch out once with ‘Private Practice‘, the franchise never fully explored the many expansion ideas that were once on the table.
In fact, creator Shonda Rhimes once revealed that a spin-off on the Shepherd family was seriously considered. But as it turns out, this idea could have done more harm than good to some of the show’s most important characters.
Why The ‘Grey’s Anatomy’ Spin-Off Idea Never Took Off

As ‘Grey’s Anatomy‘ celebrated its 20-year milestone, Shonda Rhimes admitted there were several spin-off possibilities, including versions set in cities like Chicago or Boston. But rebuilding the same world in a different place didn’t excite her enough to move forward.

Instead, she explored a show focused on Amelia Shepherd and her sisters. Rhimes shared in an interview with Entertainment Weekly that since all the sisters are doctors, the concept could have easily turned into a full-fledged series.
She even said it “would’ve been very interesting.” And, the idea sounded like a natural extension of the Grey’s universe.
However, this concept never progressed beyond the discussion stage. The Shepherd family may be important to the story, but that importance doesn’t necessarily translate into a compelling standalone show.
The Shepherd Family Was Never Built To Lead A Series

If you look at how the Shepherd family functions, you’ll understand why the idea never saw the light of day. In ‘Grey’s Anatomy‘, the Shepherd family plays an important role, but mostly in relation to Derek. His relationship with Meredith Grey is one of the show’s biggest storylines, and his journey has a strong emotional impact on viewers.

Even after his death, his presence is still felt. Amelia Shepherd also brings depth to the story. She’s talented but flawed, and her struggles with addiction make her character more real. Later, when it’s revealed that she had a brain tumor, it gives them a solid storyline and explains some of her past behavior.
However, the rest of the family doesn’t have the same level of detail. Their mother, Carolyn, is strong and respected. And the sisters Nancy, Liz, and Kathleen are all successful in their own fields. But they mostly appear to serve a purpose within Derek and Meredith’s story. They were never really developed enough to lead a show of their own.
Even if the creators tried to build a show around them, there would be some major challenges. Derek wouldn’t be part of it in a meaningful way, which would leave a big gap since he’s the most important link in the family.
Also, the sisters don’t live in the same place, so bringing them together regularly would feel forced. Unlike ‘Private Practice‘, which had a clear setting and natural connections, this spin-off would struggle to feel believable.
Honestly, the idea feels too narrow. A good medical drama needs diverse stories, patients, and relationships. Focusing only on one family, especially one that mainly supports the main story, could quickly become repetitive.
So, yes, the Shepherd family adds value to ‘Grey’s Anatomy‘, but they aren’t strong enough to carry a separate series. Keeping them within the main show was probably the best decision!
