One of the ‘Big Three’ and a fan favorite, ‘Naruto’ depicts the story of a ninja world where the strong prevails. Our protagonist fights his way through to make his presence known while meeting friends and mentors along the way. On his journey, he is accompanied by his mentor Itachi, his godfather Jiraya, and his group of friends.
Though an orphan, he meets people who become his family and finds himself a distant relative, who also assumes the position of the Fifth Hokage of the Hidden Leaf Village. Much emphasis has not been given to the relationship between Naruto and Tsunade, but undoubtedly they are related. Let us discuss a few points on how their blood connects them with each other.

Tsunade: Descendant of Uzumaki Clan
Tsunade Senju is the granddaughter of the first Hokage Hashirama Senju and Mito Uzumaki. She is the only one who carries the bloodline of the Senju clan at that time, also a descendant of the Uzumaki group. Since both Naruto’s mother, Kushina Uzumaki and Mito are from the same clan, it is assumed that they are distantly related.
Moreover, there’s an argument stating, Tsunade’s mother is the daughter of the First Hokage. Senju and Uzumaki have always shared deep bonds, be they tied to marriage or friendship. After Naruto’s parent’s death, he found a parental figure in Tsunade and Jiraya. That is also one of the reasons why Naruto calls Tsunade his grandmother.

Though jokingly enough, it is also to tease her. Tsunade is the same age as Jiraya, so she’s above 50, but her appearance states otherwise. Naruto often teases her by calling her old, but it’s also because he sees her as part of her family. Jiraya on the other hand is his godfather. Naruto’s name was derived from a book written by him. He was the ‘Child of Prophecy’, as stated later.

Tsunade And Naruto’s Relationship
Since the very beginning, Tsunade and Naruto share an impeccable sweet yet naggy relationship. After Jiraya stated that Tsunade is his age, Naruto started teasing her for keeping her young appearance. Their exchange seemed more like bickering than quarreling.
While having met with her only a few days, Naruto puts himself in front of Kabuto and Orochimaru. He took the beating for her, stating he’ll rather protect her than stand still and watch things unfold. Similarly, Tsunade got over her fear of fight and blood and protected Naruto.

She developed a fear of blood after losing her love, Dan. He died in front of her which traumatized her for life. Tsunade also lost her younger brother during the Second Shinobi War. Whoever she gave her necklace to, ended up with a tragic fate. Yet she started believing Naruto and handed him her precious necklace.
They developed a special bond, not that of a Hokage and student, but rather a familial bond, in which Tsunade saw Naruto in the image of her brother. He also carried the same Will of Fire as Dan. All these incidents led Tsuande to forget the past and move on with the present, finally accepting the role of the Fifth Hokage of the Hidden Leaf Village.
